Online Credit Application Form

To complete the Credit Application Form online - click this link  On-Line Credit Application form, or click the "Online Form" button below.  Complete all required fields.  Review the form and make any corrections.  Electronically sign the form, which simply means when prompted you will type your name and title in the appropriate fields, check a box to confirm you have read the Terms and Conditions and click the "Sign Now" button.  The application information will be sent to us and we will notify you when your account is established.

Offline Credit Application Form

To download a printable copy of our Credit Application Form, click on the "Offline Form" button below.  The form will download in PDF format.  To view and print documents in PDF format you need Adobe Acrobat Reader which may be download at www.adobereader.com .  After downloading, print the form, complete it and fax it to 775-628-7095.

Tax Exemption Certificates

To grant you tax exempt status we are required to have a current copy of the appropriate state tax exemption certificate on file.  For Texas customers, download the Texas State Tax Exemption Certificate by clicking on the "TX Exempt Form" button below.  The form will download in PDF format.  To view and print documents in PDF format you need Adobe Acrobat Reader which may be download at www.adobereader.com .  After downloading, print the form, complete it and fax it to 775-628-7095.

For New Mexico tax exempt customers, fax a copy of your Nontaxable Transaction Certificate (NTTC) issued to you by the State of New Mexico to 775-628-7095.
